Request forms

There are currently two types of requests that Data Governance handles:

1. A standard data governance request. Use this form if you have a question that involves stewardship of data, requests to use existing data in new ways, requests that involve multiple data domains, or questions about data policies.
2. An Artificial Intelligence request for either a project or service that will be used for university business. Please note that this does not include academic research projects unless the project in question is requesting to use Yale University data.
